Fellowship Program
2‑year Fellowship for Value-Based Care Leadership
- Includes sponsored MBA from Kellogg School of Management at Northwestern University
- During the first 18 months fellows will complete 6‑month rotations: Population health, Clinical Quality, and Comprehensive Care Delivery
- During the last 6 months, fellows will complete a Capstone Venture elective for credit at Kellogg and a 3‑month“interim” medical director role
- 2‑year fellows will be required to work for Oak Street Health for 2 years after completing their advanced degree or pay back the cost of their degree
- Longitudinal leadership development and senior mentorship
1‑year Fellowship for Value-Based Care Delivery
- For physicians who have already received advanced degrees
- During the first 9 months, fellows will complete 3‑month rotations: Population health, Clinical Quality, and Comprehensive Care Delivery

- Who is a typical Oak Street Health patient?
Oak Street Health is a network of primary care centers for Medicare-eligible patients. Most of our patients are 65 and older and roughly 40% of our patients are dual-eligibles (e.g., Medicare and Medicaid).
- How is Oak Street Health’s model different than other primary care practices?
Most primary care practices follow fee-for-service models, where every service is billed and paid for separately. Under a fee-for-service model, quantity is often prioritized over quality of care.
Oak Street Health operates under a value-based model, meaning we take on the risk of our patients' health — including primary, specialty, acute and post-acute care. This means we can make substantial investments in primary care services that have a positive health return. Oak Street Health pays providers more than average primary care practices, and our compensation system is more transparent. Bonuses are based on quality metrics, not R.V.U.s.
Additionally, we offer a competitive benefits package that includes dental insurance, malpractice coverage, white coats, research and reference benefits (e.g., Up to Date) and C.M.E. stipend, among others.

Our team includes internists, family physicians, geriatricians, nurse practitioners, physician assistants, podiatrists and behavioral health specialists.
- How many patients do Oak Street Health providers see each day?
On average, Oak Street Health providers see 15-18 patients per day. This compares to over 30 patients per day for the average primary care practice.
- What would my day-to-day be like at Oak Street Health?
You’ll work in a team-based environment. Typical teams include a physician and/or nurse practitioner supported by a licensed care social worker, nurse and medical assistant. A scribe with advanced training in population health and medical documentation supports providers during appointments. Transition nurses assist with hospitalized patients, and the Oak Street Health Chronic Care Team offers support for house calls and other services for our highest risk patients.
- How does Oak Street Health support personal and professional growth?
We offer a variety of development outlets including quarterly Grand Rounds, continuing medical education, affiliation with local medical schools, “morning report” style lectures from local universities and private executive coaching for leaders. We offer opportunities for residency programs and preceptorships for physicians and NP/PA students throughout Chicago. We have residency partnerships with Northwestern University, University of Chicago, MacNeal Hospital and Swedish Covenant Hospital to name a few. We offer NP/PA preceptorships with Rush Medical Center and University of Illinois at Chicago.
